I thought FSM will ask you which fund house you intend to invest using epf, then they will send the correct docent for you according to the fund house? What is what they did in my case.

U can print the forms yourself from FSM website.

It's only the EPF form with pre-printed Fund House code that u cannot print from FSM website. Aiyo, just print the EPF form with blank code, then when u decide which Fund House, just call FSM to get the code and u fill it in by pen lor.
