Volume 3. Actually it's still a little hazy on how his mother died, but most likely she died while shielding Ichigo from Grand Fisher. Hell, since his father is a shinigami, who knows? His mom probably could see Grand Fisher too and knew that was a hollow.

Well, I think the author is trying to reveal as little as possible because I smell that the moment Ishida recuperates completely, he'll be fighting with Ichigo and gang. Or something similar. Might be nice to see if he plans to play along to his father's instructions or backstab him. That one will be an interesting story arc by itself.
