a lot of factors contributing to FC, not only on foot nor high rpm. It depends on the road condition, road gradient, road traffic, humidity, air density, air temperature, engine displacement, type of engine oil used, engine condition, tires (type, width, diameter), tires condition, COD (co-efficient of drag), vehicle weight and total volume in the cabin (weight of occupant/cargo). All these plays a role too in addition to "right foot" and rpm.

when engine running lean, no matter what is the condition, it will produce extra amount of heat. So, yes. prolonged lean burning will damage the engine.
