The Ghost army bits is add-on by Fran Walsh and peter Jackson, although walk through the haunted mountain pass was in the book.Additionally, in the book, the Elves army didn't come to help in the Two Towers, again, an addition done by Fran Walsh and Peter Jackson.
In my opinion, for the PG-13 movie, the horrors of war were quite well portrayed in the movie (decapitated heads being catapulted into the city of Gondor comes to mind).
