basically, it is two way interaction between the device and the user, in order to maintain higher degree of awareness/motions, basically soul(brain circuit) is "connected" to the device. For activities with health risk, the major concern is how to tone down the "pain"/damage transfer from machine to human brain, in order to prevent brain self-induced damage to own body.
Its emergency bail-out system apparently still incomplete and require external aid to launch it, but still it is way better than Matrix ones.
Brain-hack is another issue raised in Ghost In The Shell anime.
