Money changers would be available at airport.
Do take not that money changers over there are the bank themselves.
If you have USD, shoul be no problem fot yens. But Im not sure if they accept MYR.
alternatively you can always opt for advance-cash using creditcard for some cash at Maestro tagged banks

The visa validity is 3month. so your jp trip must be within the period the visa is given.
On how long a stay you can get on the visa, i have no idea. The longest i got was 15days (my trip was 2weeks)

considering taking the new Skyliner or NEX, 1 way to Shinagawa or ueno around 45mins.
I's say you got 2hrs to roam in the city itself.
