I "think" that these 5+ achiev are only done if you start a new campaign in that difficulty, not if you just replay 5 missions in Hard from a Normal campaign for example.

I know I first played a Normal campaign, completed it and then replayed 1-2 missions in Hard and it wasnt counting.
Starting a new campaign in Hard made them count.

I am not sure, I did not pay super duper attention to it so I could be wrong

I can confirm that it does not need to start a new campaign. I replayed missions [in different difficulty] in the Mission Archive and got these +5 achievements.

I confirm that too.

I completed a mission in Hard obliviously without thinking what other achievements that I am going to get other than what is displayed at the end of the "summary". Then I noticed an achievement is grey-ed out and went on to get it done without proceeding further.

After that I just got a Hard 10+ achievement
