A Word of Warning
While most wireless carriers offer you an “unlimited” data plan for the duration of your service contract, it is quite common for there to be small print that protects the carrier from excessive usage on your part. It is always a good idea to discuss any plans of this nature with your service to ensure that you are within their terms of service. You do not want to get a surprise when you receive your bill the following month. These words to the wise also apply to anyone who is not currently using a plan that allows for unlimited data transfer. If you do not have unlimited data plan, do not even think of using your BlackBerry as a modem.I have followed step-by-step instruction on BB Bold setup as modem (shown in the spoiler below) and copied the BB Bold modem script, everything working fine and configure except the APN setting.

I have merged the step-by-step instruction and APN settings for both PC and Mac setting up for BB Bold 9000 as modem for tethering via Bluetooth (Also attached BB Bold modem script, only applicable for Mac users, as for Window PC, it will automatically installed the driver)

Here are the tethering instructions and files needed to tether the Blackberry Bold to a Macintosh Computer through Bluetooth:
1.) Extract the Modem Script from the attached .zip file and place it in the Mac HD > Library > Modem Scripts folder. Be sure you place it in the root Library folder and not the Library folder associated with your User folder.
2.) Pair your phone with the Mac. Be sure you have set it up as a Trusted Connection and is set to "Yes" and not "Ask".
3.) In Preferences, select your phone in the list, and then click "Configure this device". This will launch the Bluetooth Setup Assistant.
4.) Check the "Access Internet..." and proceed.
5.) Select "Other" for Vendor, and "Blackberry Bold" (name of the modem script) for Phone Model. Leave the Username, Password, and Phone Number blank. Continue.
6.) Launch Network Preferences, and click "Bluetooth" in the list.
7.) Select "Add Configuration" from the Configuration drop list. Name the configuration something you can identify. I use my carrier name (Maxis).
8.) Populate the remaining fields with your APN information, using the wap server URL as the Telephone Number.
Example for Maxis :
Telephone Number:unet
Account Name:maxis
Password: 123
9.) Check "Show modem status in menu bar"
10.) Click Advanced, and verify the information is correct.
Modem
* Vendor = Other
* Model = Blackberry BB Bold
* Enable Error... = checked
* Dial Mode = Wait for dial tone before dialing
* Dialing = Tone
* Sound = On
DNS
* -All Blank-
WINS
* -All Blank- (NetBIOS may be populated and disabled)
Proxies
* Configure Proxies = Using a PAC file
PPP
* Settings = Session
* Third, Fourth, Fifth and Seventh items checked. Redial 1 Time, and Wait 5 Seconds before redialing
That's it. You are ready to tether.
Enable Bluetooth on your phone, and then on the Mac. Click the modem icon from the menu bar, and select Connect Bluetooth. After a few seconds you should see a connection timer and you are connected.

First you must satisfy these criteria:
A. PC is using the Microsoft Bluetooth stack (to verify this, look at the entries under Bluetooth Radios in Device Manager).
B. The BB is paired with the PC.
C. BB Bluetooth is turned on and device is set to Discoverable.
D. Bluetooth Dial-up networking service is enabled on the device (on your BB, go to Options->Bluetooth->Options and look under Services).
E. In Device Manager, under Modems, there is one instance of "Standard Modem". In its properties dialog, under the Modem tab, the port number corresponds to the first RIM Virtual Serial Port (under Port in Device Manager) number installed on the system (should be fine as long as Desktop Manager is installed).
After you verify the above, then you can proceed (Upon pairing Bluetooth and BB is done) :
1. Double-click the Bluetooth icon at taskbar select BB under the Devices then click Properties. Or if you don't see Bluetooth icon at taskbar, go to Device Manager, select bluetooth, right click to Properties.
2. In the device Properties window, go to the services tab and take down the COM number beside the Dial-up networking (DUN) field.
3. Create a new connection using the New Connection Wizard.
Click on Start -> Connect To -> Next
-> Set up a connection or network
-> Connect using a dial-up modem
-> Select the Standard modem with the COM which is same as COM number in Blackberry under Bluetooth device list -> properties under service tab!
4. Unselect others field -> Next -> Give a random ISP name -> Next -> leave the phone number field blank -> Next -> leave all user name and password blank -> Finish
5. Enter *99# in the Dial field
6. Go to Control Panel -> Phone and Modem Options -> Modems, select the modem with the COM number obtained in step 2
then go to Properties -> Advanced. In the Extra initialization commands field, input the following string then click OK: +CGDCONT=1,"IP","net" (can either be "net" or "unet")
7. Connect *connection name* windows click Dial, leaving the user and password field blank.
9. On the BB, Accept incoming Bluetooth connection from PC.
