GCE O-Levels are for your regular curriculum and yes, are accepted. however, the SAT 1 is a must for everybody wanting to attend university in the USA.

No. TOEFL or IELTS is required for international students coming in from countries whose native language is not English.

undergraduate in Japan will require fluency in Japanese because all classes are taught in Japanese. if you're doing graduate studies in Japan, then you'll have English taught classes, but even that depends on the university. I know ToDai offers graduate degree classes in English
