i applied cimb petronas. Formed downloaded from internet and filled in details nicely.. brought the form n docs to the branch n the officer told me that.. internet form no use 1.. then she get an ori form, attached it together with the downloaded n just need a single sign. she said she will copy the details for me.. 1 weeks later card receieved.. hehe

maybe they will credit the points later?
citibank is very slow in giving the extra points too, take 2-3 months to get the 2x points
