3.Is your media (blu ray movie, PS1, PS2, PS3 game, cd audio) in a good condition?

4.How long usually you play games/movies per week?

5.Did you take out the media after the session?

6.What do you play mostly? Game for PS1, PS2 or PS3? Movies DVD, movies blu ray, audio CD?

7.Are you experiencing extraordinary heat from your PS3 regularly?

8.What is your PS3 model? 20/40/60/80

*for PS1 and PS2 games, this will only involved games with media, not bought from PSN, for stock 60,20 and 80 gb machines.

so far, I can see one of the factors of this problem is playing pirated stuffs..

i cant remember where I read, but that is actually the BD drive locking up..not sure tho'
