I recently switched service providers from Maxis to Unifi, and they provided me with the Skyworth Combo AX3000.

Take the POLAM serial and pwd
Phone verifications related info (use inspect element fuction from your browser)
Get a huawei HG8240h5 unifi version, key everything in and you have a n ONU to replace that combo box
The reason for using HG8240H5 is, you can change the serial number of the ONU to the same serial of the combo box
