From the wording, seems like some kind of aircraft failure. But have to be careful when releasing the final report. It will affect all similar aircraft around the world and manufacturer's reputation as well. So all covering asses with verification with manufacturer for confirmation. Ding dong ding dong. But looks like not pilot error.

All aircraft parts installed must have the related certificate and fit for for release to service as per CAA Malaysia
