The only difference is process. UHT and pasteurized. There is no such thing as fresh milk fresh from tits. Just either reconstituted or not.
Pasteurized is more expensive solely because it needs refrigeration.

(or fresh from cow tits if you are adventurous enough?) afaik there are no gazetted definitions for fresh. it could be non-reconstituted or non-processed aside from sterilization.
