Probably the instructor don't expect the girl able to temporary lifted the weight, thus removing it from safety of the the hook holding the weight.

This type of gym failed and knee bending and betis patah I cannot stand to watch too long. Feel cannot tahan, if video head crash or accident head tercabut I'm okay with that.

The big guy next to her was a client, not her spotter...he was just there...but who set up the heavy bars? Why she simply sit down without saying anything?

Probably sharing the equipments.
Jack guy finish 1 rep and trying to add weights. See him like want to take the small weights at the side.
Maybe he thought the girl was fooling around and didn’t think that she will try to lift it.
From the direction of the seat, the guy is probably benching, if squat, it should be other direction so you can fall and to the floor instead of getting stuck at the bench.
The mother want show off to daughter maybe, and then died.
