This UMA.. how does it look like? Do I need to fill in anyting?

The UMA is just a file to download after you have complete all the registration online.

From what i understand seems like i need to send this uma pdf plus ic plus bank account statement to the email?

You just reply to that email that ask for it. The UMA, IC and Bank Account statement (for bank in purpose).

Yes

Make sure your IC is certified in JPN
And your bank account statement has to go to the bank to print one, not from the internet, ask the staff its for tuntutan wang tidak dituntut, its free.
