Apart from the lain2 issue i think the main issue is facilities and money

You put specialist in a rural hospital without facilities, they can't do anything. That's why there is a skill discrepancy in those areas manned by only nurses etc

Yeah unfortunately

If the place have the facility at least the sick patient don't have to travel but the doctor can visit on certain occasions
