I would recommend Beastars but it is a bit heavy.

Also, I admit that I am obsessed with it and I have rewatch the anime over and over again as well as re-read all the manga chapters over again.

almost all anime have heart wrecking scene
coupled with sad ost, even funny anime gintama also can do it
but if you want constantly heart wrecking depressing anime:

fafner in azure (it feels like nge neon genesis evangelion hopelessness with gundam seed arts)

for reverse isekai, i would suggest the devil is a partimer.

other isekai that other may missed:

1. Zero no Tsukaima
2. Mondaiji-Tachi Ga Isekai Kara Kuru Sō Desu yo?
3. Drifters (one of my favorite opening theme there)
4. The Philosopher's Grandson
5. High School Prodigies Have it Easy Even in Another World
6. Isekai Cheat Magician

i would recommend for u to watch 1,2 and especially number 3 since its one of my personal favorite isekai anime

now 4,5 and 6 is not really that great. but i guess for ur quench of isekai anime, it should be okay for u.

6. Ano Hi Mita Hana no Namae wo Bokutachi wa Mada Shiranai (Anohana) ---> damn i like the story!
7. Plastic Memories
8. Love, chunibyo and other delusions - the last part of season one.. its actually a romcom but there's a tear jerker there too
9. barakamon ---->its heartwarming but at the same time can be a bit sad.... mostly a heartwarming but i put it just for u to watch
10. bakuman ----->3 seasons if im not mistaken. and u should watch all of it.
11. white album
there are others that tear jerker anime but i only remember those mention above.... now get ready ur tissues because its going to be a roller coaster of emotions lol....

erm i only watched clannad after story before from ur list..is clannad n clannad after story the same thing or? i googled macam diff ish?
