Normally HR screen is the behavioral screening - this one you need to stay calm and answer the question in honesty. Dont show your frustration when answering, else will be the delisted as a candidate regardless how good you are in technical. HR screen

Senior Programmer + Hiring Manager + hands on (coding test) this is done in the same session during face 2 face.
CEO - this is very rare unless your position is direct reporting to CEO in a big corp or the company is very very small.
