 Need Urgently, Pediatrician recommendation

Our 1.5 month baby has been having reflux all the while. For past 3 weeks, we are always woken up by his stretching and grunting followed by spitting out milk and saliva, then coughing and crying. We made sure he s burped after drinking milk and hold him in more sitting position. After 1 hour plus only we would put him down into his cot. had tried elevated his matress a bit with towels underneath it. Still the same.

We had been to govt and private hospital but all dismiss it as normal for newborn especially premature baby.

Yesterday hit the worst. Totally cannot put him on any surface - baby cot, changing mattress or bed. He will vomit milk soon after put him down. He vomit, then gasp for air for a few second, though his face color does not change. He even cough out milk during feeding but when we stop feeding him to let him rest a while, he will keep crying for more milk.

Baby hasn't sleep much since yesterday, looks very tired.

We are at loss. I had taken care of younger siblings and nephew before. They vomit milk, but not this way. Wifey is in a premature baby facebook support group. The mothers in there are the only one who are not brushing the symptom off and urge us to seek other doctors fast.

Appreciate if anyone can recommend a very good pediatrician. We are sick of going to hospital and told it's normal when we have to quickly set him up and put his head face down when he chokes. Wifey is so scared that she's carrying him all day long, which seems to lessen his stretching and reflux.

How much milk u feed him? Latch?
Newborn milk should be very very little only.
Feed less but frequent. Normal to cry for milk every hour.

Not sure if doctor can do anything. Good luck bro. Take care.
I would only sleep him in carry mode. Sacrifice a bit lo as parent

Is there any helper grandma or nanny to help taking care of your baby. As your baby is premature and on top of that having reflux, it must have been taken a toll onto both you and your wife...

Parents need to get enough rest as well especially your wife so do employ and get someone to help out to offload you and your wife

As you mention, you already consult few Drs. If you have consult 3 or 4 including both government and private, I think there is no need to seek others. There is very little diagnostic that the Pediatrician can run at this time. If the situation did not improve after 3 month old, maybe the Dr will look more closely into the baby digestive system and etc.

So... stay clam, get enough rest for both you and your wife.

60ml every 2.5 hours. Pedi recommended 60ml every 3 hours but try as we may he will get hungry 2.5 hour.

Bottlefed with breastmilk but sometimes supplemented with formula when not enough milk stash. We tried fully latching before but couldn't monitor how much he drinks and he always seem not full after latching. Always cry after latching, when he pulled himself away.

What should we do when he cries for more milk after drinking 60ml?

Carry mode lessens the issue but does not make it away. sometime when we changes position (due to numb hand), reflux again.

Wifey thinks he will die from choking if we leave him in the cot to sleep.

We consulted but doctors didn't do much examination. We just hope to consult one more experienced pedi, just some more examination or possibly give some medication if it's acid reflux.

Also baby's breath smells very sour. After vomit he always sleep with weird noise like he's singing/mumbling.

I think 60ml every 2.5 hours for premature baby is alot. maybe thats what causing the problem. cried after latching doesnt nessessarily mean still hungry. you can also try using pacifier

let baby decide how much to drink. not by measure. as he is premy, digestive system may not fully develop. dont think doc can prescribe any med. even have med also risky. the reflux is his natural way of defence to keep alive.

suggest not to do formula and fully latch if possible. latch more frequently.
during latching if baby unlatch and cry, maybe could be cannot breath. checked nose? or other factor? too hot? too cold? diaper?

Few things,

1. If milk is from mother’s one, make sure mother eats non gassy foods. Even some vegetables are gassy.
2. Change the formula if that’s not suitable for your kid. Check the stool color and change the formula to lesser strength. For examples some kids Enfalac is higher strength so much heat it produces may lead to stomach upsets
3. Change the bottle to Tommy Tippee or Dr. Brown. Both the bottles are less airy than other bottles.
4. Try not to use the pacifiers.

Had an episode this morning again when milk came out from nose and mouth, then gasping. We have to hold his head down because he arched his back so much and choke even more. Quickly took a video of the situation, went to a pediatrician and showed her.

She told us vomitting milk is normal but not like this until gasping for air. She don't think we are overfeeding but baby has bad reflux. Were asked to try enfalac+ar milk which is for anti regurgitation, plus one medication. With AR formula though, we know we have to monitor his poop in case constipated.

Bottle feed or not, latching or not, formula or not... we had tried all. In fact he is worse when latching. He latches non-stop, but in between, keep pulling away, cry cry cry, then searching for breast again. Then drink until choke and cry again. Once, he latches so long at midnight until wifey dozed off and both almost fell off chair. We prefer bottle feed so I can help too. During confinement, my wife spent so much time exclusively latching until showing signs of depression.

Back to all saying 60ml being too much for preemie, isn't baby still rooting, searching for breast with his mouth and putting his hand into his mouth means he's still hungry? If we feed him less than 60ml or feed him only 3 hours later, he becomes very upset, crying non stop until his face red and body feels warm. Sometimes he does take 40-50ml only, so we let him be. But most of the time, he will calm down at 60ml. Just sometimes at odd hours he will ask for latching. How we know that? If I am holding him, he will turn to my chest and bite.
