Phone not accepting PUK code

My tunetalk is asking for PUK code but when i enter it it says incorrect code. I have checked with tunetalk and confirmed the PUK code is correct. Also tried with another phone but same error. What could be the cause?

If you attempted more than 10 times, it will always reject PUK code. The only solution is to change SIM card.

Maybe u miss read 1 of the characters on the PUK?
Happened to me once though. |
