System did not include EIS amount in the total. And the word 'or' implies one only, not both. 
I think everybody will just max out this by SOCSO + EIS.

May I know whether can claim tax relief of RM1.5k per parent, if there is no medical bill?

This used to be allowed but now it has combined with the total 8k of medical cost for parents.
