1. Yes, there are many convenience stores (Familymart, Lawson, 7-11, Seicomart) with toilets inside the store along the main roads. And of course, they sell food and drinks. Sometimes, you will find family/tourist parks along the main road where there are toilets in it. Drive slowly so that you don't miss it. The road signboards (indicating there are toilets in the park - using the normal symbol of man and woman) are usually obvious and clear.
2. I even bother to use Waze in Japan. No jam.
3. Yes, but not during winter. So, I'm not sure how is it like driving during winter. Anyway, the speed limit can be as low as 40km/h in some stretches.
I used pocket wifi when i was in Hokkaido. So, I'm not sure about SIM card.
