if a/l, a/p, bin, binti is in your ic or passport you need to include it in formal documents.

not formal ones you can skip it.
Not formal, you can and drop Kok Yee as well.

First Name: Alan
Last Name: Tan

not many forms got middle name options.

Some forms instead of first name, last name they put first name, surname to take in consideration of multicultural naming convention.
