1. Firstly make sure you've completed followup in hosp.
2. Make sure all MC photocopied and countersigned
3. Then go to SOCSO office tell them u wanna claim and get the claim form
4. Fill up ur details and submit to Pejabat Rekod in the Hospital
5. After 2weeks - 1 month they would have filled up the form which u have to submit back to SOCSO office
6. After about 1-2 months they will have a meeting involving you, your employer, your company medical advisor and the hospital Specialist
7. During this meeting the compensation will be determined.
