The application form for double storey

Hi, would like to check where can I find the application form if I want to build my single storey house to double storey? I know we need to get approval from MBPJ, but is it possible to get it through online and to find out what are the procedures?

There is no virtual forms for you to download.
You need to get to their office personally, queue for the number and request. The officers will guide and as well, inform you on the payment required to be made.
You need to prepare quite a lot of documents as well such as the plan to the building.

Don't expect to just make one trip and get everything settle  Go there the first time, get the forms, make your inquiries and jot them down in your small little notebook. Go back home, compile the necessary documents and comply with the conditions and then go and submit on your second visit. I believe you need some documents to be certified as true copies by a lawyer or someone qualified.

Or is there any place I need to go before heading to Majlis?

So definitely need to travel few times before I can finally submit all the documents. That is just their "way" of doing things.
