this one u give the officer to scan

then they will stamp your arrival card
proceed to immigration as usual

u still need fill white card TM6
QR code is for health authorities to check u fully vaxxed or not

Just found out TM6 not needed if you arrive via any of Thai international airports.

Of course accepts card, with 20-30% surcharge (Agogos). If normal restaurant/7-eleven can use card. I use ApplePay there.
Moral of the story: Just pay cash.
