Yes, GST should also be charged on the entire total inclusive of any fees.

I agree with you that the credit card fee should be included in the sales price to you but since it wasn't, GST covers the entire amount of goods and services charged to you unless there are any zero-rated or exempt items in the invoice.

1. You can cancel the card and after some time (6 months to 1 year, depending on bank), you'll be considered new to bank again. If you are sure you didn't sign a credit card application form, you can complain to bank and BNM about this.

2. Based on my personal experience with several banks, the loan officer will include an empty credit card application form and ask you to sign when applying for home loan. Most people will just sign it and the loan officer will fill up the form for them.

Should be RM47.50 assuming all petrol spending was at Petron using the card.

Rapple's example does not take into account the compounding nature of credit card interest, you will be negatively surprised if you follow his example. It's really not hard to google for a decent credit card interest calculator online.
