Most of the insurance policy/ premium you pay for, likely to cover for local incidents/ hospitals. Check with your agent on this.

Travel insurance also cover aspects such as natural disaster (eg. you're in Japan and earthquake happens), it covers your hospital bills, loss of belongings etc while your insurance premium may not cover (again, check). Tour must charge because they do not want to be liable for anything that happens to you.

Besides, you pay for premium each month so do you make claims every month? Appear meaningless as well when you don't fall sick or get hospitalised and claim back the money. Right?

I guess because that's a necessary document. Thus did you manage to claim? You can always contact the airline and check - see if they can email a copy to you.
