hi all master, need advice for renovation permit

i just bought a single storey house (20x65') small landed house in PJ ,
because of the house is so small , i decided to demolish 2 rooms , relocate the bathroom and the build a room on top (convert to 1.5 storey)

my question is:

1) how much is the cost for drawing and mbpj permit submission ??
2) what doc do i need to apply permit ??
