when you reach the counter, they will told you that your current form only for 1 phase.
to change to 3 phase, you need to have contractor to check your current house board and incoming cable...
to ease the job, just use the TNB appointed contractor..
i have mine done last year September...
including deposit total charges including contractor = RM1.8K
