I am not confuse with the item 1,2 and 3 as what you have stated.
I am confuse of payment method, should be pay by card (deduct card money) or pay by ewallet (deduct app money)

Pay toll using added card mean deduct card money, right?
Or as stated in their website, "The physical Touch ‘n Go card that has been added into the eWallet will be used as a medium to pass through all “Touch ‘n Go” and “SmartTAG” toll lanes." which the card is just a medium to access to ewallet and the toll fares will be deducted from app.

You can their hub at NU sentral which allow you to print a card with your photo.
