this signed declaration letter when making OD-KFK? what is it really?

Some said only keputusan report + police sketch will be enough?

Somebody bang my car from behind last week. Dented my bumper and bonet.

Made police report within 24 hrs (i think he did also). But I have yet to send in my car to workshop because of the long holidays. Going to do it tmrw and probably get the keputusan report from balai
