Can anyone provide me a sample of statutory declaration form for Loss of Use claim that is to be signed by the commissioner of oath?

Hi guys, I am trying to make a 3rd party claim for loss of use. However, one of the requested doc from MSIG is final police investigation result. Only initial investigation result is currently available from the police station as the sergeant told me that he is unable to proceed with the final investigation report as the other party did not make any police report. I did provide MSIG this initial report, however, MSIG will only proceed with the claim with a finalized investigation report and not initial. Any advice?
