I usually topup at Johor Mydin and the last topup is on Sunday, no issue. I just purely do TnG topup no, groceries purchased. Maybe the staff is not well trained or their POS system is lousy. It always happened and I usually have to wait for quite some times.

Go take Rapid KL and see.  I think at least at KTM, you just still need to scan once and it allows to pass through.

RM 10 not RM 18.  This happens if you scan and didn't hear the two times beep. If you tap again, gone case. I always pray hard when I use KTM. Luckily, I take LRT all the time. Thank God.
