Campaign Mechanics:-
i. 12 months Instalment Plan-Interest Rate at 0% per month.

As for the form, I think you better wait for them to send to you. In my first email, I already attached latest 3 months of payslip and current year EPF statement.
Then, they will reply you with the form.
