External or internal ? Normally I just use PowerAmp to play music, it's default EQ + N5's clean audio output is good enough for pop song. Currently still testing and tweaking V4A. Using Brainwavz M4.

Try reflash first. Reflash wont wipe your data. Just flash the ROM again, wipe dalvik cache, and reboot.

Definitely fit Slim Armor. But be careful, it's tightly fit. I applied mine with the Slim Armor on.

Might be the sound of OIS component. The vibrator itself is quite tiny, not sure if it could make voice it bit.
Are you on custom Rom or had been modify something that makes vibration louder/stronger? Coz I did modify my vibration to be slightly stronger so I'm not missing notification.

If you are going to reflash Rom, after done flash the Rom, boot into it first, then only goes back recovery and flash other kernel.
