I know it's a filler but did it really take place in real world or just a dream? If it was just a dream how come Kouta from the real world could fight the same monster that appeared in Kaito's dream?

Kouta needs to learn when it's time to man up. I get that he is good guy, but there's only so much time you can let Micchi walk all over you. If Kouta starts to spit nonsense about saving Micchi again the next episode, I would lose lots of respect for him.
