Just did this today, so wanted to update for anyone having their items detained by customs.
The forms (borang) that was originally attached by op is still the same forms used today so download them, fill em up and head to Block 25 in Sirim Qas Seksyen 2 shah alam. Really friendly staff and you'll get it done in less than an hour if tak ramai org.
For faster process, please print (and fill up) the form that was forwarded to your email from pos Malaysia, please print the specs of your product (ask from your seller / print from the item's website), and prepare the RM100.
Wait for a few hours and you'll get your permit emailed to you. Now head to Pos Malaysia International Hub, Jalan KLIA S1, with your permit, invoice and anything you think might be relevant.
For me, I ordered a japanese phone from Kyoex. Smartphones are 'conditional prohibited goods'. Since GST 6% no longer exists, you'll have to pay SST 5%. Calculate readily and bring extra cash (in case of any extra payments) cuz although some people said can use card, sometimes system down lmao
