Chapters come out every week. Chapters are only grouped into volumes after all the chapters in the volume are released.

best guess would be that Isshin changed his name so nobody recognizes the name Kurosaki. and Ichigo doesn't bear any resemblence to Isshin, only to Kaien.

plus most of the captains are new ones (relatively) so they probably wouldn't know much about Isshin anyways.

I believe the similarity of Ichigo and Kaein is just nothing more than a coincidence. The story was just made up such way.
