what are your cpu/hdd/memory/gpu loads/utilizations? please have a logging/benchmark running in the background to gauge

also your fps value if it is constant or sudden spike/dips etc.

so this is a spike and not constant right? ok if you're on vista/7, under computer management (assuming you're at least on pro/highest version, not sure if the lower versions has such tools) there should be a performance category where you can see more details on your pc performance (way more than task manager) and there should be a portion for hdd I/O utilizations, gauge that. check when you spike, is the I/O high.

i believe vista and above, you can customize your task manager to include disk I/O tab as well on the processes tab (in addition to cpu/mem usages, and task IDs/user or system IDs and process name along with desc.)
at least that's the case for my vista ultimate smile.gif

now if all of that is proven to be true, chances are you may be incurring high disk I/O and causing the lag spikes (do realize that d3 has no loading screens, it is all done in the background CONSTANTLY and they dont care if you're fighting an elite or not, if you're near an exit, it will start pre-loading the exit's levels/files)

additionally,
is your OS 32 or 64bit? what are your physical ram utilization after 15-30mins of gameplay on d3?
what are your windows pagefile settings?

if your RAM utilization are low and you still face this issue (assuming it is confirmed to be disk I/O issue), you can refer this thread:

please do not do that as some driver/graphic card will get stressed out trying to achieve max fps, gets over heated, auto throttling and you actually get shit fps.

my point is still valid though, and i have never encounter fps drop badly when having fps limited for 1 of my PC and 2 of my laptops (1 laptop being sis, another sis playing on my laptop)

that's 3 machines in total mind you.

not limiting it on the other hand, let's just say it isnt the friendliest thing to majority of notebooks and it probably won't be very "lap"top friendly

this is the bug.
if it is an issue, use 3rd party software to limit fps, or have vsync to limit it to 60.

tldr:
- you will need limitations on fps so your graphic card doesnt stresses itself up pumping frames you may not need
- if your fps below the limit anyways and you still suffer from low fps despite having everything low with the d3prefs modification, it's just your machine not meeting requirements
