I downloaded from the web although I did went to Japan Foundation KL once to inquire about it.
Just didn't have time to go browse around it's library so much.

Happy New Year is usually mentioned this way, right?

akemashite omedetou gozaimasu.

or shorten form for more informal

akemashite omedetou.

I'm also going for JLPT this year.
Gonna try to take Level 3 and Level 4.

@crazy25kid
Which level(s) are you taking this year?

Seems they're gonna revise JLPT at 2010.
There will be a new level between level 3 and level 2 to bridge the vast difference in difficulty between the 2 levels.

Application Form costs RM5.

TEST FEES: Level 1 (RM60), Level 2 (RM60), Level 3 (RM50), Level 4 (RM50)

REGISTRATION PROCEDURES:
1. 2 recent photographs (3cm x 3cm passport – sized);
2. Application must be accompanied by a photocopy of the applicant’s I/C or passport;

We need photocopies of IC? I never had that.

Added on July 24, 2008, 9:37 amActually, you can skip levels. My fren skipped level 4 and took level 3 straight.

Well, that's what it states in Japan Foundation Kuala Lumpur.

Yes, it's not compulsory to take the lower levels.
You can straight head for Level 1 if you have the confidence.
