When i was 30 year-old, i 'failed' my blood test for down syndrome. Test during 4th mth of pregnancy. That mean, the percentage for my age to get down syndrome baby is much higher than usual ppl. This make me really tension.
Then, need to do further test (Amniotic fluid test) to confirm whether the baby is ok. Then result of Amniotic fluid is - the baby is normal. Yes, my baby then borned normal.
Q: Now, i'm 34 year-old and pregnant, since my 30-yr old down syndrome blood test is failed. Will this time failed? Or there would be chances to pass the test? because when i was 30 also failed, now 34...
Q: What is detailed scan? What can it shown? My doctor only do down syndrome blood test without the detailed scan. Doubt she dont have the 'machine' in the clinic? Is necessary to have detail scan?

Hi, Congrats on your pregnancy. I do not know the answer to the first question but for the second, a detailed scan was done at the same time of the blood test to check the nose bone and spine at the neck. My doctor checked that the nose bone of the fetus is straight and the measure,ent of the one at the neck. If these two criteria is met, then the down syndrome risk is down by 70%. Thus, no need to worry so much while waiting for the result of the blood test. The machine he used was quite advanced. Can do 3d scan one. Hope this info is useful. You can visit FMGC in TTDI PJ for this scan. Quite pricey. It cost us 600++ two years ago.
From what I was told, the most accurate test would be First Trimester Test with NT, nasal bone and blood test (sorry if I messed up the data, you can visit the website for exact details). It can be done before your pregnancy exceed 14 weeks.
I also having second thought. My wife gynae suddenly asked if we wanna do double test. We did ask about the First test but he was like it doesn't matter. I rather do the First because its 95% accuracy compared to Double or Triple Test. But I can tell you, the specialist at FMGC very pro. Instead of scaring you, he was quick to calm you down and look at the positive side. Like one of his patient with 1/20 DS but he said his patient got a healthy baby. We tend to look at the negative side too much until overlook the chance for a healthy baby is 19/20.

Its been 2 years++ so I cant really remember which is which, but we did blood testing, plus some ultrasound test which includes measuring some of parts of the fetus which can only be done before 14 weeks. All of it cost us RM600++ but that was two years ago. Now surely dont know what magical number.

Thanx for the info. Btw, what is Nuchal scan? FMGC - can i know whether is there any good Lady gynae to recommend? FMGC- do u know rougly how much is the delivery charge or is there any package?

I am a 32 year old mother of one and currently am pregnant again.
1. I am not aware of what you mentioned, blood test for down syndrome. Can blood test really test down syndrome rate?
2. I went for amniocentesis during my first pregnancy because we missed the nuchal scan. It was a risky experience but we were first time parent then so we went for it.
i) Nuchal scan is a kind of scan where the doctor measure the back of the neck of the baby. They will measure it 3 times and then take an average. Anything below 20 is safe from having down syndrome child. However, this is only 70 - 80% accurate, I think.
ii) the amniotic fluid extraction test, I think is called Amniocentesis is where they put a needle into your tummy to extract the fluid out to go through a series of test. This is about 99% accurate.
Note: Usually, down syndrome test is only required for age 35 and above. Most of the time, nuchal scan will do for most parents.
3. Detailed scan is where the doctor scan for any abnormality in the baby. It will scan the baby brain, heart, kidney, arms, hands, fingers, thigh, leg, foot, toes, etc. It is accurate to a certain level.

The "triple test" measures the levels of αFP and two pregnancy hormones called unconjugated estriol (uE3) and free ß-human chorionic gonadotropin (ß-HCG). It is called the "double test" if two of the substances are measured and "quadruple test" if four substances are measured. These tests can detect between 65 to 77% Down syndrome if it is done between 15-18 weeks of pregnancy. In other words, they will detect two of three Down syndrome but will miss one out of three.
More recently, the first trimester screening tests have become increasing available for mothers in Malaysia. This involves taking a small amount of blood to measure the levels of two substances, free ß-HCG and pregnancy associated plasma protein-A (PAPP-A), which is normally found in all pregnant women. This blood test detects 68% of Down's syndrome. This test is usually done in combination with an early ultrasound examination, which measures the nuchal translucency (thin layer of fluid usually found at the back of the fetal neck) and the presence of the nasal bone between 11-13+6 weeks of pregnancy. The combination increases the detection to over 90% of Down's syndrome. What about ultrasound scan? Recent studies showed that ultrasound measurement of the nuchal translucency (an area of fluid beneath the skin of the fetal neck) and presence of the nasal bone will detect almost 90% of Down's syndrome. Since it is "non-invasive", there is no risk of miscarriage. Eleven to 13+6 weeks gestation is an ideal time to measure the nuchal translucency (NT) and look for the presence of the nasal bone. An increased nuchal measurement and absence of the nasal bone increase the risk of Down's syndrome. If this is the case, a cvs or amniocentesis is advised to check the baby's chromosome.
More recent studies have combined ultrasound with first trimester biochemical markers to give up to 95% sensitivity or detection rate of Down's syndrome.

You can try NACE test. Its safe and affordable. NACE (non-invasive prenatal test) test is 100% safe for you and your baby. It is as simple as a routine blood analysis. A small blood sample taken from the arm that is sufficient to run the test. NACE test is a new type of genetic test that screens for birth defects and inherited diseases. The NACE is to detect birth defects such as Down syndrome, chromosome abnormalities, genetic disorders.
