Historically there have been alot of instances.

in knock-out stages, where winners have to be decided one way or another, then last (not preferred of cos) method would obviously be by chance eg coin toss.

just curious - how come win one drew one still have to take penalty kicks?

or u referring to a league format and the results is not home/away but between different teams? hmm
