The only bank that I can think of if you wanna have your name embossed on a debit card is Public Bank or Citibank Malaysia, also the withdrawal fee is RM 10.60 per withdrawal for Public Bank Petron Visa Debit Card but with that card it is "annual fee FREE".

The Citibank Debit Card might have an annual fee and it is also RM 10.60 per withdrawal and 1.25% on top of every transaction. Note that the 1.25% is standard across all banks in Malaysia.

Nope, I forgot whether CIMB allows up to RM 5,000 or RM 10,000 daily limit and NO MORE than THAT.
