There is no official proportion. Hence, epf and life is not limited to 3k each. Can be either 4k EPF or 2k Life or vice versa...

Q1 - Yes if ur annual income falls below the bracket. However, do you entitle to bonus on your 2nd company?

Q2 - Form B for sole proprietor. BE for employment. E for Sdn. Bhd.

They will notify you through letter.

Last, i received was 14 working days after my submission.

The point is, it is the duty and responsibility to DECLARE your income.
From the declaration & calculation, it will determine if the income is taxable or not.
If not taxable, then no need pay anything. Keep the documentation for future use such as house loan, car loan, credit card application, etc.

This is pretty normal. Don't expect high efficiency from LHDN especially on refund.

Based on your information, i assume your annual income has yet to reach the level. Hence, you do not need to declare your income.

FYI, my ex-company hr does provide EA form to all employees irregardless whether they are tax payers or not.
