if you are self employed, by right you don't need to pay at all. anyway, you may pay as much as you like since the amount you paid are not tax deductible. However your Employee portion is deductible at the personal relief in the Form B

refer to the address of the IRB from the letter, you may choose to post or dispatch your dividend voucher & HK3/HK6 to the branch. they are working on 5 day week.

amount claimed would exclude Government Service Tax. Anyway, your claim is max to RM500 per year.

do a tax computation to show that whether are you taxable. if it is not taxable, it is up to them whether they want to audit you & check the supporting documents

on top of that, if you file in your tax returns every year, it would be weird that you lapsed 1 year. it is advisable that you keep all your personal income tax records

by just earning 2.1k per month. IRB won't even register an income tax account for you but you must keep your own EA Form issued by employer
