I think the Manchester thing applies mostly in Malaysia and places outside UK, because city was virtually unheard of before they got promoted and become a premier league regular so by calling it Manchester, ppl knows u are referring to Man Utd. For inter/Milan, both of them have been the big name in Italy for a long time, calling inter as Milan will be quite misleading. Though i more prefer calling the club Inter Milan and AC Milan in full rather than just Inter or Milan. For me, calling one Inter and the other Milan is not justifying the fact that Inter is from Milan as well (like calling MC as Manchester and MU as United).
