Use disk scan to check for your hdd error. Run CHKDSK to check and fix your HDD error.
But with the symptom of 2nd and 3rd information that you had provided, i can tell you that your HDD have a huge bad sector and its almost dying.

Your HDD is almost dead.
My idea would be:
Take your HDD out from the casing, use a Notebook HDD SATA converter to run it in a PC see if there is possibility the hdd still can be accessed.
Thanks

Hello,

I am having problem with my harddisk partition right now. The scenario goes like this. I have two harddisks (40GB and 250GB). I boot from the 40GB, while I store all my stuff in the 250GB. Both harddisk are using NTFS. Recently, I reformatted the 40GB for fresh clean install of Windows XP SP2. When I boot into windows, the 250GB was shown as unpartitioned. In a nutshell, my secondary harddisk suddenly became unpartitioned after a fresh format. Since it's a clean install, I am sure I didn't install any software to mess that up. My secondary harddisk is the WD2500SB.

Just an extra note:
Previously (few years ago), I've posted this in the forum before and someone (sorry I forgot his name) came up with a simple solution by asking me to post a HEX data of my unpartitioned harddisk. Then he asked me to change a value so that the partition becomes 'identifiable' by Windows. According to him, for some unknown reason, the 'identifier' of my harddisk was changed. So windows identified that partition as 'unpartitioned' instead of the actual 'NTFS'. This method managed to recover all my files. Unfortunately, I searched up and could not find the post anymore. Anyone can help me with the problem?

Was that me? I also can't remember you.
I can help with MBR-style partition table, FAT, NTFS troubleshooting and data recovery using hex/disk editor.
As for the tools to use, I haven't choose the best to introduce. Hex Workshop works great as a plain disk editor, but later I found WinHex which is more suitable for this purpose. Unfortunately, WinHex trial version is severely function limited.
