USJ is pretty good.. not sure about Fuji Q but i heard that Fuji Q roller coaster is popular.

Re-entry permit is only valid until your visa expired date. I am not sure does tourist visa allowed to do Re-entry permit or not. and you need to pay to do it.. 3000yen for single Re-entry permit, 6000yen for multiple Re-entry permit.

and this is for my experience and I am not holding tourist visa.

if you mean to stay another 3 months, means that you need to re apply again for the visa.
